This optional template can be used to develop information that explains how and when performance data will be collected on the performance indicators a Mission included in its CDCS. It is an aid for preparing a set of Performance Indicator Reference Sheets. A table of this type usually starts with the CDCS Goal measures and then sequentially presents the indicators for each DO and all of the IRs and Sub-IRs associated with that DO. The kit includes information on how to fill out each column of this table. The template can serve as an internal worksheet or it can be included in a PMP. Additional rows can be added as needed.
